Fine Needle Antegrade Pyelography of Transplanted Kidney

Abstract

In a poorly functioning renal transplant, ureteral obstruction or perforation must be considered as a cause. Sonography and radionuclide scanning will demonstrate outflow obstruction and/or pararenal fluid collection, but do not usually provide sufficient information on the site of the lesion. In these cases we have successfully used fine-needle puncture of the renal pelvis under sonographic guidance and antegrade pyelography. Superior anatomic details of the renal pelvis and ureter are provided by this technique. No complications due to fine needle puncture have been observed.
In a poorly functioning transplant kidney, postrenal causes must be excluded before the diagnosis of rejection crisis is made. Sonography is the method of choice to exclude ureteral obstruction but cannot define the exact site if present. Retrograde pyelography is often technically difficult due to the abnormal insertion of the ureter into the bladder(i). We present our technique of antegrade pyelography and discuss its safety and possible application.
